Information for Your First Visit
We ask that you bring with you the patient demographics, medication list and history forms already filled out. If you have records from a previous physician, we ask that you bring those as well. On your initial visit we will review your medication and history forms with you prior to your visit with the physician. ALL information shared on these forms remains confidential.
